Intraoral scanners. Facial scanners. CAD libraries. Artificial intelligence. Digital smile design. High-translucency lithium disilicate. Yet none of those technologies can answer the most basic question: <strong>What exactly are we trying to achieve?<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">My blunt view is that many disappointing dental veneers are not fabrication failures. A serious veneer case needs measurable targets for the face, teeth, gingiva, optical behavior, and function. It cannot prove that the patient will speak comfortably with them.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">That is why I distrust static-photo planning when it is used alone. A natural-smile video often reveals lip asymmetry, head posture, muscular compensation, and tooth display that a carefully posed photograph hides.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">A study examining <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/21195262\/\" rel=\"nofollow noopener\" target=\"_blank\">smile esthetics from the layperson\u2019s full-face perspective<\/a> reinforces the broader point: esthetic variables are judged differently when the whole face is visible rather than only the teeth and lower face.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Patients buy a face-level result. We should design one.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Goal 2: Establish Tooth Hierarchy, Not Mechanical Symmetry<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Perfectly equal teeth look wrong.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The central incisors should usually command attention. Lateral incisors should support them rather than compete with them. Canines should turn the smile into the posterior corridor instead of appearing like two additional central incisors.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">This is visual hierarchy.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Central Dominance Must Be Deliberate<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">For each central incisor, define:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Planned length<\/li>\n\n\n\n<li>Apparent width<\/li>\n\n\n\n<li>Facial line angles<\/li>\n\n\n\n<li>Incisal edge form<\/li>\n\n\n\n<li>Contact position<\/li>\n\n\n\n<li>Embrasure development<\/li>\n\n\n\n<li>Degree of symmetry between the two centrals<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">Do not rely on a universal golden proportion. Faces are not spreadsheets, and tooth dimensions that work for one patient can look absurd on another.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The more useful question is this: <strong>Which teeth should the eye notice first, second, and third?<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">That hierarchy can be controlled through line angles, visible width, length, value, texture, and embrasure progression. A technician can make a tooth look narrower without making the physical tooth dramatically narrower simply by moving the reflective line angles inward.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">That is real porcelain veneer design. It is not dragging a CAD boundary.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Decide Which Asymmetries to Correct<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Facially driven smile design does not mean forcing every feature into mathematical equality. It means identifying which asymmetries distract the eye and which ones make the smile believable.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Controlled differences in incisal wear, lobe expression, surface texture, or lateral-incisor rotation can create life. Random differences in central width, gingival height, or incisal plane create disorder.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">My rule is simple:<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Correct structural asymmetry. Preserve selected biological character.<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">A patient asking for \u201cperfect\u201d veneers may actually want clean alignment and consistent color, not eight identical ceramic blocks. The mock-up consultation should separate those ideas before the final design is approved.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Goal 3: Decide What the Gingiva Will\u2014and Will Not\u2014Allow<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Ceramic cannot negotiate with biology.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">If the gingival zeniths are uneven, papillae are deficient, recession is active, or the tissue architecture conflicts with the planned tooth dimensions, the problem must be identified before veneer case planning moves into final design.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Otherwise, one of three things happens:<\/p>\n\n\n\n<ol class=\"wp-block-list\">\n<li>The technician creates long or bulky cervical contours to disguise tissue discrepancies.<\/li>\n\n\n\n<li>The clinician requests a remake for an asymmetry that existed before preparation.<\/li>\n\n\n\n<li>The patient discovers that \u201cnew teeth\u201d did not produce the gingival frame shown in the simulation.<\/li>\n<\/ol>\n\n\n\n<p class=\"wp-block-paragraph\">None of these is a ceramic problem.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Record the Tissue Architecture<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">The pre-design record should identify:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Gingival margins tooth by tooth<\/li>\n\n\n\n<li>Zenith locations<\/li>\n\n\n\n<li>Papilla fill<\/li>\n\n\n\n<li>Recession<\/li>\n\n\n\n<li>Black triangles<\/li>\n\n\n\n<li>Biotype<\/li>\n\n\n\n<li>Crown-lengthening history<\/li>\n\n\n\n<li>Tissue inflammation<\/li>\n\n\n\n<li>Planned periodontal or orthodontic intervention<\/li>\n\n\n\n<li>Timing between tissue treatment and final scanning<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">The team must also decide whether tissue asymmetry will be corrected, visually disguised, or accepted.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">State it plainly.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">A digital rendering with idealized gingival scalloping can become a dangerous sales image when the clinical plan does not include a way to produce that tissue position. I consider that less a communication error than a consent error.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Control Emergence Without Creating Bulk<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">A veneer can alter facial contour, but it has limited authority over the cervical environment. Feldspathic porcelain can offer delicate optical layering and surface characterization. Zirconia, ZrO\u2082, may provide greater masking or mechanical options in selected cases.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">But material selection should follow the optical target. A single unretracted shade photo taken under a ring light is not enough.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Cross-polarized photography can help separate internal color from surface reflection. A calibrated shade tab in the same plane as the tooth helps. Over-glazing can raise apparent value. Incorrect line angles can make a central incisor look wider. Excessively smooth ceramic can look synthetic even when the shade coordinates are technically close.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Natural enamel, built largely around hydroxyapatite\u2014Ca\u2081\u2080(PO\u2084)\u2086(OH)\u2082\u2014is not an optically uniform tile.
